Upon entering the office, please inform the receptionist at the reception counter of your arrival. If this is your first visit, you will be asked to fill out a general information sheet and a medical history sheet. Please do so as accurately as possible, since this information will allow your ophthalmologist to focus on possible eye diseases associated with the respective medical conditions, if any. All medical records are kept strictly confidential. After completing the form make yourself comfortable. If you need to use the restroom facilities, please ask the receptionist.

For the benefit and comfort of the other patients, please do not smoke in the waiting room. Tobacco smoke aggravates certain eye conditions. Also, we respectfully request that you turn off your cell phone and refrain from using your cell phone while in the doctors offices.
